Pick the one Deductible Amount under which all family  members will be covered.  Then select your semi-annual rate from the chart.   There are four rates to choose from. You can insure: 1) Yourself only; 2) yourself and your spouse; 3) yourself, your spouse, and all unmarried dependent children under age 19 (or 25, if a full-time student); or 4)  yourself and all your children.
