Only you know the right amount of monthly income you need if you become disabled.. and the amount of protection you can afford to purchase.
With this plan, you can apply for as much as $5,000 in monthly benefits in multiples of $100... Not to exceed 60% of your basic monthly pay.*
Choose a monthly benefits package that fits your needs as well as your budget by following these simple steps:
- Multiply your current monthly paycheck by .6 to calculate how much insurance to buy.
- Select either Plan I (5 years of Benefits) or Plan II (Benefits to age 65). NOTE: Disabilities which begin between ages 63 through 62 are payable for only 2 years and disabilities which begin at ages 60 and over are payable for one year or to age 70, whichever is earlier.
- Then multiply the amount of monthly benefit you want by the semi-annual premium per $100 in the chart below.
- Send no money now. We will bill you later. Your effective date of coverage will be the first day of the month on or following approval of your application and receipt of the first semi-annual premium by the insurance company. If you need help calculating your premium, call the Plan Administrator.
* Benefits may be reduced by other disability benefits to which you are also entitled. Your total disability benefits from all sources may not exceed 70% of your basic monthly pay

| How to Calculate Your Monthly Benefits |

Assume you are between 50 and 54 years old and earn $5000 a month. You are eligible for up to 60% of your basic monthly pay (.6 times your salary) or $3,000. You've selected Plan I. The chart above shows your semi-annual premium is $11.34 for each $100 of benefit or $11.34 times 30, or $340.20 semi-annually plus, $2.00 billing fee. |
Premiums are based on your attained age and will increase as you enter each new age bracket. Maximum monthly benefit: $5,000.
New York life reserves the right to change premiums on any due date and on any date on which benefits are changed. Future benefits are subject to change by agreement between New York Life and the Policyholder.
