Select one which applies to each family member to be insured under this plan.  Then look up the semi-annual rate in the chart below.   You can insure: 1) Yourself as Member, only; 2) yourself and your spouse; 3) yourself and all your unmarried dependent children under age 19 (or 25, if a full-time student); or 4)  yourself, your spouse, and all unmarried dependent children.
